NOTE:  The PSI bulletin is in error.  You should be using a 2017 version of Ugly‘s (not a 2008) and a 16th Edition of the American Electricians‘ Handbook (not a 2002).  This recommendation is based on requirements for other states using the same NEC version.

NOTE:  Reference #3 above is a 1994 version of NFPA 72 that is out-of-print.  Should be the same as the version (2016) referenced in other electrical exams like Systems Technician Part I.
